The Villages Health Hospitalist Program

Play Video

Hospital care can be complicated and even overwhelming when you’re sick and trying to recover. As a patient of The Villages Health (TVH), if you’re hospitalized at UF Health Villages® Hospital, you’ll be admitted by a TVH doctor. We perform surgeries and procedures requiring hospitalization here, where our TVH specialists have privileges. A TVH hospitalist will oversee and coordinate your care during your stay. A TVH care management nurse will also help coordinate the appropriate care both during your stay and after your discharge.

UF Health Villages® Hospital shares the same Electronic Medical Record (EMR) with TVH so our team can access your medical history, test results, and treatments at any time. In addition to the TVH team members we have overseeing your care at the hospital, this shared medical record helps with care coordination to ensure the best health outcomes for you.

Overseeing care so that nothing’s overlooked.

To ensure you receive the best care available, our hospitalists see only 8 to 12 patients per day, rather than a traditional hospitalist, who sometimes sees as many as 35. Our hospitalists meet our strict requirements, have access to your complete medical records, and maintain regular contact with your primary care doctor.

The right follow-up ensures better health outcomes for you.

Upon your admission to UF Health Villages® Hospital, a care management nurse is in contact with you both during and after your stay as needed. Your care management nurse is available to contact your primary care team and other providers, as well as help you understand and keep track of all necessary follow-up measures. These may include rehab, specialist visits, and home care – details that often get overlooked in a long list of post-discharge instructions. Our goal is to make sure you don’t end up back in the hospital.

Meet the team.

Each time I visit I become more impressed with how well the Center is organized, staffed and operated. I've had several visits to Audiology with Greg Freeman over the past few weeks and found him to be reassuring and professional and encouraging, making my transition into hearing aids a little easier to deal with. Everything from check-in, the visit itself, the follow-up appointment scheduling and the check-out runs like clockwork.
