Health Coaching

Have you been contemplating a change in one or more of the following areas?  Do you need an accountability partner to guide you along the way to improving your overall health and well-being? Our Health Coaching program can help with

A Health and Wellness Coach acts as your personal GPS to help you discover the possibilities through incremental lifestyle changes that promote a holistic approach.  Health Coaches partner with patients to identify their own goals for change, helping them to facilitate the self-discovery process for sustainable lifestyle changes.  Coaches help patients overcome the struggles, and empower patients for “real” life.  Rather than putting every patient on the same plan, they work with individuals to learn their strengths, weakness, and goals to create realistic and achievable goals.  You may discover the reasons you want to change on a deeper level, empowering the change. 12 weekly sessions, 90-minute initial visit and eleven 45-minute sessions, $200 fee

Satisfied and Slimmer Plant Based Weight Management Program

Satisfied & Slimmer is designed for people who have difficulties with long-term weight management.  You’ve done it all…..reduced carbs, reduced fats, increased  exercise.  Nothing seems to work!  You are persistent at weight loss because it’s important to you.

We have a lifestyle approach that may be a better fit for you to lose weight and be healthier.  You may also find that you are more satisfied and no longer have urges to eat unplanned foods like sweets and salty treat foods.  This is all while eating carbs and healthy fats. Moreover, no calorie counting involved!

How can this be? The strategy is to eat more foods with lower calorie density that are more filling.  These are foods with more fiber and water such as fruits and vegetables.  We will also guide you on how to eat more healthy carbs and fats.  While we will be asking you to reduce your intake of animal products (meats, dairy), you do not have to become a vegan. We show you how to get adequate protein intake. We will give you specific guidance on how to incrementally change your food plan and eating habits.  We will address barriers to implementing a plant-based food plan.

In addition, behavioral strategies for successful lifestyle change and weight management will be reviewed.  These include: urge management strategies, mindful eating, getting social support, and responding positively to setbacks.

7 weekly 1-hour sessions; $70 fee (Discounted rate of $55 each for participants that sign up with a friend) 

Cognitive Behavioral Therapy for Insomnia 

Cognitive-Behavioral Therapy for Insomnia (CBT-I) was designed for people who have difficulties falling asleep, staying asleep, or awakening early. The 4 session program provides an understanding how our behaviors may inadvertently be making our sleep problems worse, how to prepare our body for sleep, how to build the skills and techniques to reduce unhelpful mental activity (worry and racing thoughts) and cope with strong emotions that affect sleep, and how to troubleshoot any problems and refine strategies to further improve sleep.  Education on sleep medications will also be provided. This approach is experiential.  For it truly to work, participants practice strategies in their daily lives.

Grief Group

Loss is life changing. It can affect so many aspects of our lives and ourselves – identity, lifestyle, responsibilities, support, ability for joy, ideas about ourselves and the world. Grief is like a river – ongoing & often changing, it can be full of turbulence, flow, rapids, and unexpected fluctuations. Sometimes grief is more complicated, sometimes totally different than we might have expected. Navigate your personal grief experience in a supportive space with others who have experienced loss.

Gain insights into:

  • Grief myths and stigmas
  • Managing social interactions following loss
  • Coping with changes to lifestyle or responsibilities
  • Regaining sense of purpose and satisfaction in life
  • Managing intense feelings or meltdowns
  • Ways trauma complicates grief

The purpose of this group is to find validation, support, awareness, and to alleviate a sense of isolation that often accompanies grief. Join us for support in moving forward with grief toward remembering with more love than pain.

Mind-in-Motion™:Movement Therapy for wellness, empowerment, fitness and fun!

Whatever paths we have walked in life, our bones, fluids, muscles and cells hold a “memory” of our experience. Stress, grief, trauma, joy: Our emotions affect our bodies, determining how we move through the world…and our bodies are a powerful part of how we express emotion. Research shows that expressive movement can help reduce anxiety and depression, boost self-esteem and even improve sleep, by reducing stress hormone levels and restoring the body to a calm, natural state. Mind-in-Motion™ uses fun, energizing, dance-based movement to improve emotional and physical wellness. Join us to discover a more nurturing way to inhabit the body you were born in—come and learn to “dance through life!”

Matter of Balance

Did you know that 1 in 3 older adults fall each year? Many older adults experience concerns about falling and restrict their activities. A Matter of Balance is an award-winning program designed to manage falls and increase activity levels. Join us and learn to view falls as controllable, set realistic goals to increase activity, make changes in your home environment to reduce fall risks, and exercise to increase strength and balance.

Memory Screenings

A memory screening is a wellness tool that helps identify whether possible changes in memory and cognition are age-related or the beginning of a more serious memory problem. It is not intended to diagnose but instead to evaluate how different areas of your brain are working, providing a baseline of brain function over time. Since many factors triggering memory loss can be reversible, it’s important to practice early detection so that symptoms may be treated.

Dementia Care Coordination Program

The Villages Health offers a Dementia Care Coordination Program to primary care patients that are dealing with Alzheimer’s and other Dementia disorders. The program consists of one-on-one meetings with one of our Certified Dementia Practitioners and small group sessions. Caregivers attend eight weekly education and support sessions while their loved ones participate in an interactive respite program.

Prevent T2:

One out of three American adults has prediabetes, and most of them do not know it. Having prediabetes means your blood glucose (sugar) level is higher than normal but not high enough to be diagnosed as diabetes. This raises your risk of type 2 diabetes, heart disease, and stroke. Without weight loss or moderate physical activity, many people with prediabetes can develop type 2 diabetes within 5 years. The year-long PreventT2 lifestyle change program, developed by the Centers for Disease Control and Prevention, can help you make lasting changes to prevent type 2 diabetes.

Year-long program: classes meet once a week for 16 weeks, followed by every other week for 2.5 months, and then monthly meetings for the remaining 5 months; $100 total cost ($50 material fee and $50 incentive fee).

Choose to Lose:

Choose to Lose is a specially designed weight loss program for those with a Body Mass Index (BMI) of 30 or higher. Led by a Registered Dietitian, this small group format covers topics such as Learning to Eat Well, Shopping and Cooking, Succeeding Away from Home, Eating Mindfully, Seeking Support, and Keeping the Weight Off.

This is a year-long program: classes meet weekly for the first month, every other week in months 2-5, and once a month for the last six months. Participants also have two one-on-one sessions with the Registered Dietitian; Cost: $150

Diabetes Education:

More than 1 in 10 Americans has diabetes. Lifestyle habits, including healthy eating and exercise, have a significant impact on reducing the risk of complications from diabetes. The Villages Health offers the only American Diabetes Association Recognized Diabetes Education program in the region. It is a comprehensive program for individuals with type 1, type 2 and gestational diabetes. A Certified Diabetes Educator and one of our Registered Dietitians will work with you and your primary care physician to develop a personalized plan through one-on-one appointments and small group classes.

Medicare, UnitedHealthcare Medicare Advantage and Florida Blue Medicare Advantage cover Diabetes Education with little or no out-of-pocket cost to you. Commercial insurance and Medicare Supplement plan coverage varies. Our team will verify coverage before booking your appointment.

A referral is required for Diabetes Education. If you are not a Villages Health primary care patient, we will send you the paperwork to be completed by your provider.

Medical Nutrition Therapy:

The Villages Health provides Medical Nutrition Therapy to individuals that have Diabetes or Chronic Kidney Disease stage 3 and higher. Three hours of one-on-one Medical Nutrition Therapy with a Registered Dietitian is provided in the first year of education and two hours of one-on-one Medical Nutrition Therapy are available every year thereafter.

Nutrition Counseling:

Meet one-on-one with a Registered Dietitian to review your health history and develop a customized meal plan that works best for you. Nutritional Counseling is available to everyone, regardless of age, insurance or condition.
