CenterWell Senior Primary Care (Vitality), Inc., together with its affiliates (“CenterWell”), has an arrangement with The Villages pursuant to which it is allowed to use webpages located at certain Villages domain names, including this webpage, for limited purposes following the closing of a transaction on November 7, 2025 whereby CenterWell purchased substantially all of the assets of The Villages Health. CenterWell is further permitted as a licensee to use certain The Villages Health trademarks.
