How Does it Work?

• Prior to your visit, you will receive an email or text if you have forms that need to be completed or information that needs to be updated for your upcoming appointment.

• Simply follow the links provided to fill out your forms whether you’re at the care center, at home, or on the go! Complete your forms with ease right from your home computer, tablet, or smart phone.

• Opening the provided link will prompt you to fill in your date of birth and zip code to verify your identity.

• Once verified, you will be directed to the forms and questions applicable to your visit.

• You will fill these out, submit them, and then… you’re done!
