Dementia Care Coordination Program – Interest Form on Tuesday December 31, 2024 at All Day

The Villages Health offers a Dementia Care Coordination Program to primary care patients that are dealing with Alzheimer’s and other Dementia disorders. The program consists of one-on-one meetings with one of our Certified Dementia Practitioners and small group sessions. Caregivers attend eight weekly education and support sessions while their loved ones participate in an interactive respite program.

Free for TVH patients/caregivers. $150 for non-TVH patients/caregivers.
